Diabetes Insulin

For type I diabetics, insulin is essential. It’s administered subcutaneously by the patient, following medical indication and, in very special cases, by a doctor (diabetic coma). There is fast-acting insulin and slow acting insulin. Of the rapidly acting insulin ordinary or regular crystal is used mainly. It’s important and it’s best to use purified insulin.

As a general rule, the patient will inject insulin, 3 times daily, 15 minutes before each main meal (morning, noon and evening). Doses are determined by the amount of carbohydrates in each meal. There are mandatory rules:

-1 Insulin Unit (IU) of regular insulin, about 2 g glucose balance. If the patient’s urine contains 20 g glucose in 24 hours, he/she will, therefore, need 10 IU. If after 10 IU, glycosuria disappears, the patient can be considered balanced. In general, but especially in children and adolescents, the allowed glycosuria has a value of 10 -12 g; in 24 hours it’s best to avoid possible hypoglycaemia.

Children and teenagers are not supposed to receive treatment with slow insulin, or, at least, it should be administered in a single daily intake. Store the vials of insulin in the fridge and always keep them in a dry place. Before administration, the vial must be brought to room temperature and stirred to homogenize. Insulin extraction is done with two needles, sterilized by boiling, which are inserted into the rubber stopper (after disinfection with alcohol and erasing with a sterile cotton plug of water, then drying). Alcohol inactivates insulin. Extract the insulin through a needle and through the other inserts the air that will replace the first needle’s contents, which is extracted. It is desirable to discard the syringes after each use. There are 1 ml syringes with gradations 40 (every gradation corresponds to 1 IU) or 20 gradations (each gradation corresponding to 2 IU). Sterilize syringes only by boiling. If possible, sterilize the syringe once (in the morning), by boiling and, with it, a double number of needles compared to the doses of insulin injections per day; also have a triple number of wool caps.

Injections are subcutaneous. In general, injections in the same place must be avoided. Change the injection location; this will be done regularly and following a scheme that is made by the patient, who also makes his own single injection.
